• Luminare Health Screening Information for Families

    Uplift Families -  

    As we get closer to our scholars starting their first day of in-person or hybrid learning, we want to remind you of what needs to take place every day before your scholar arrives on campus, as well as what they need to bring to school with them every day.  Each day, you will fill out a symptoms questionnaire through a web-based app called Luminare.  Please read this entire email to learn how you will use Luminare each morning and as part of our carline procedures.

    As a reminder, below is the schedule by grade level for scholars returning for in-person or hybrid learning.

    September 21- PreK and Kinder

    September 28- In addition to PreK and Kinder, 1st, 2nd, 6th, 9th, and 12th

    October 5th-  All scholars who selected either in-person or hybrid as their learning options.  Those who selected “remote” as their learning option will continue to do so from home.

    Symptom Awareness & Daily Health Screening 

    In order to ensure the safety and well-being of all our Uplift scholars, staff, and families, we ask that you please have your scholar(s) stay home if they or anyone else in your immediate family are exhibiting any of the following symptoms: 

    • Fever or chills 
    • Cough 
    • Shortness of breath or difficulty breathing  
    • Muscle or body aches  
    • Headache  
    • Sore throat 
    • Congestion or runny nose 
    • Upset stomach 

    I want to emphasize to always wear your mask while on campus, emphasize the importance of social distancing, and wash your hands frequently.  When all individuals are wearing their masks properly (mouth and nose covered) AND are socially distanced, the risk of spreading COVID (even if one person is positive) is dramatically reduced.  Hence, we are asking our staff to be vigilant about social distancing and mask usage for ourselves, with each other, and with our scholars.  Together, we can reduce the spread of COVID on our campuses! 

    Scholars and families will be able to let us know of any symptoms proactively through the use of Quickscreen by Luminare, a web-based app that enables users to self-check for COVID-19 symptoms in themselves and their household.  

    Quickscreen by Luminare, is not something that you will have to download but instead, it will send you an email with a link to the questionnaire to the address we have on file. If you are wanting the email to be sent to a different email address, please contact your school to have it updated. 

    Families can also use a QR code to access the questionnaire. If you do not have a QR app downloaded, simply open the camera on your smartphone and hover it over the QR code from one of the posters you will be able to find at your campus. Click the link that pops up and fill out the questionnaire.

    Both staff and scholars must ‘pass’ the self-check screen before being allowed on campus. Completing the questionnaire takes less than a minute and can be done on your computer, tablet, or smartphone. After you have completed the questionnaire for your scholar(s), it will provide you with communication on what to do. A "green" response means you are good to go and you can come on campus. A "red" response means there are potential COVID-19 risks present and you will be asked to keep your scholar(s) home, log in to the virtual learning platform, and an Uplift team member will reach out to you, check on you, and provide additional guidance for a safe return in-person.

    The health screening software gives us confidence that we are doing our part to help safeguard the health and safety of our scholars.  


    We are taking every precaution we can to minimize the health risk to our scholars and staff, but we need the partnership of ALL of our families to make our return to in-person and hybrid learning as safe and successful as possible. 
    We ask that every family follow the checklist below every day.

    Check your scholar’s temperature if you have a thermometer 

    Wash your hands thoroughly with soap and water for at least 20 seconds. 


    1. Your favorite well-fitting cloth mask.  
      Learn more about mask guidelines and tips here.

    2. Fully charged Chromebook if you have one on loan from Uplift 
    3. Hand sanitizer if you want your own.  There will be plenty in the classrooms, too!
    4. Water bottle filled with water, as water fountains have been converted to refillable bottle stations.
    5. A smile and a great attitude!

    We miss our scholars dearly and cannot wait to have them back on campus so that we can effectively serve them, and they can receive the high-level of education they are used to.
