Online Health Screening
For the 2020-21 school year every scholar, staff member and visitor entering an Uplift building will be required to complete a health screening questionnaire and undergo a temperature check. In order to speed up and simplify this process, we will be using Quickscreen, a web-based app, for the health screening questionnaire.
Quickscreen by Luminare enables users to self-check for COVID-19 symptoms in themselves and their household. Users will not have to download anything, but instead, will receive a link in an email or will scan a QR code to view the questionnaire.
The questions will take less than a minute to answer, and you will receive an immediate response. A “green” response means you are clear to get your temperature checked and enter the building. A “red” response means you may not enter the building, and further instructions will be given within the app.
Scholars and staff may complete the questionnaire from home each morning prior to arriving at school. For more information, view the video and additional resources on this page.
Frequently Asked Questions
What if I don’t receive an email with the link?
The email is automatically generated and sent to the email address on file with Uplift. Please check with your school’s front office to make sure your email is up to date in our records. You may also scan the QR code on this page to access the health screening questionnaire.
What if I get a “red” result when I do the screening?
A “red” result means that you did not pass the screening for one or more reasons and you may not enter an Uplift building. The app will give you further instructions on what to do.
If I am a scholar and get a “red” result, will I automatically be counted absent?
No, you may log in virtually and complete your daily classes and assignments on line. However, if you do not feel well, then please rest and contact your medical provider.
Do I need to do the health screening every day?
Yes, the screening must be completed every school day prior to entering an Uplift building. It can be done at home prior to coming to school, or as you are waiting in car line.
How will the school know whether or not I passed the health screening?
A daily screening with a “green” result will also reveal an “animal of the day.” When asked in car line, tell the car line monitor what the animal of the day is, then the scholar will have his or her temperature checked. If the temperature is normal, scholar will be cleared to enter the building.
If you get a “red” result, the app will automatically notify your campus. A staff member from the campus will reach out to you for further info.
