• School Medication Policy

    If your student needs to take medicine at school...

    Pick up a Medication Administration Permission Form and/or an Asthma Action Plan from the nurse’s office. This form is to be filled out and signed by the child’s physician or parents. The original prescription bottle serves as doctor's orders and can be used in lieu of a physician's signature on the permission form. With a completed form on file in the nurse’s office, we will be able to administer either prescription or non-prescription medications to your child as directed by their physician.

    All medication, both prescription and non-prescription, must be in the original container with the original or the pharmacy label in place. Your student’s name will need to be added to the container of any non-prescription medication before being left with the school nurse.

    Medication can only be administered in the nurse’s office. No student is to take medication, either prescription or non-prescription in any other location on campus.

    The exception to this is the student who has written instructions from a physician to use an inhaler or an epi.
